We want your knowledge and expertise!Job Summary and QualificationsCASE MANAGER REGISTERED NURSE - FULL-TIME - DAY SHIFT 8A - 4:30PThe Case Manager RN is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. What you will do in this role:
  • Facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team
  • Coordinates activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization
  • Provides case management services for both inpatient and observation patients as assignedWhat you need to be successful:
    • ADN required / BSN preferred
    • Texas RN license or compact license is accepted
    • Must have at least two (2) years of experience in case management -OR- three (3) years of clinical RN experience
    • InterQual experience preferred
