Registered Nurse Case Management , oversee, strategize, and monitor
the team to provide quality services efficiently. Develop
relationships, manage teams, and monitor daily activities to ensure
compliance, quality, and cost targets are met. Create partnerships
with various stakeholders to improve member health outcomes through
case management services. Essential Functions of the Role -
Collaborates in the creation and execution of strategies aimed at
population wellness. - Initiates and participates in improvement
projects spanning multiple departments, to ensure the highest
levels of quality. - Manages the day-to-day operation of the
organization's healthcare case management programs, aligning them
with established parameters. - Certifies content is current and
clinically appropriate in case management programs and meets
regulations and accreditation standards. - Supervises to ensure
quality outcomes and effective medical management through regular
performance review and auditing of work products. - Represents the
organization at meetings. Is a subject matter expert. Attends
internal clinical, quality, and regulatory meetings, and community
partner meetings. - Proposes innovative ideas for program design
and efficacy, incorporating annual evaluation and development of
work plans to facilitate performance improvement. Key Success
Factors - You should have a deep understanding and prior experience
in managed health care along with relevant clinical experience. -
The ability to work autonomously and with significant results
orientation. - Efficiency at juggling multiple tasks while still
adhering to time constraints. - Effective at communicating through
speech and writing. - Superb social skills that help you interact
with a diverse set of people. - The capacity to make efficient
conclusions dictating the process of your work. - The capability to
use overarching goals to designate the best use of resources. -
Business ability encompassing areas such as planning, budgets,
benefits, compliance, and risk management. - Basic computer skills
extending to Microsoft Office, scheduling software, and email. -
Experience in program development or project management is highly
recommended. - Having a case management certification is a plus.

degree in Nursing. - At least (5) five years of work experience is
required. - Registered Nurse in Texas.
