With a patient-centered approach and evidence-based treatments, we ensure optimal care in the comfort of home.Role DescriptionThis is a part-time or full-time role for a Nurse Practitioner with Wound Care experience at Sano Wound Care. The NP will provide specialized wound care to patients in their homes, including assessing wounds, developing care plans, and coordinating with other healthcare providers. The daily tasks include dressing changes, patient education, and monitoring healing progress. The role requires thorough documentation of patient care and communication with the healthcare team to ensure optimal treatment outcomes.Qualifications

• licensed Nurse Practitioner
• Must be licensed in Texas
• Experience in Nursing and Patient Care
• Experience in Wound Care
• Specialized skills in Wound Care
• Experience in Home Care settings
• Experience with Medicare
• Excellent communication and documentation skills
• Ability to work independently and remotelyResponsibilities Conduct in-home wound assessments and develop individualized treatment plans Apply skin grafts and other advanced wound care treatments as needed Manage and treat chronic wounds, pressure ulcers, diabetic wounds, and post-surgical wounds Educate patients and caregivers on wound care management and prevention Collaborate with physicians, home health agencies, and other healthcare providers Document patient progress and treatment outcomes in compliance with healthcare regulations
